Economic Stimulus Payment Instructions

Print these instructions and follow them when you fill out the form linked below.

Make sure you have Adobe Reader on your computer. If not, you will be prompted to go to www.adobe.com and download a free copy when you try to open the PDF document.

Open the fillable PDF document linked below.

You can type in the blanks! So go ahead - type your information in all applicable fields and see for yourself.

When you click in a checkbox, a checkmark appears. Click again, and it disappears. DO NOT check any of the boxes associated with red-lined text.

DO NOT fill in the red-lined fields. These fields are for income-tax purposes only, not for stimulus payment purposes.

When you have filled in all the blanks that apply to you, print two copies of the completed form.

Sign and mail one copy, and keep the other for your records.

Optional: While the form is open, go to File - Save Page As, and save an electronic copy on your home computer for future reference. Unless you have the full (paid) version of Adobe Acrobat on your computer, you will be informed that the file is read-only. That is fine - save a read-only copy.
