Valet VITA Consent Form

Thank you for using Valet VITA

If you are married and filing jointly (MFJ), both you and your spouse must be present during the Valet VITA appointment as IRS requires signatures from both taxpayers. If your spouse is unable to attend the appointment, you may print and both sign Part III of the Valet VITA Consent Form found below and bring the consent form along with all required tax documents to the appointment.

Although not necessary, if you would like to expedite the interview process you may print and complete the intake sheet and bring it to your appointment.
